Client Forms

 

Thank you for choosing Heart Journey Christian Counseling, LLC for your counseling needs.  In order for us to get started, please download, print and fill out the following forms.  It is imperative that you complete the required forms before coming to your first session.  It allows you to begin your session without feeling stressed or rushed, and it also ensures that you are not using your paid session time to fill out paperwork.  DO NOT SIGN YOUR COMPLETED FORMS UNTIL YOUR FIRST APPOINTMENT!!!

 

Please bring the following to your initial appointment:

1) Completed initial paperwork

2) Inner Healing Prayer application (if applicable)

3) Copy of picture of identification

4) Guardianship paperwork (if applicable)

5) Contact information for any professional or support people you would like to be involved in therapy 

 

Please note, all couples need to complete separate paperwork.

When you come to your session you will need to bring three forms: Adult or Minor Client Information, Informed Consent, Bank Card Authorization (if paying by card). The Notice of Privacy Practices is for your review only.
